*Exact pricing will be provided to you prior to beginning your plan. Please note that because each care plan is different, these are estimated amounts. Active care plans are typically more than wellness/maintenance plans.

Family and Military-First Responder Plans Available

*Active care plans average a course of 3-months, although can be longer or shorter depending on extent and length of condition. This is determined after your examination. Once the active care plan has been completed, a wellness care plan maybe offered. Wellness plans typically start at $71/mo. Care plan options are only available with self-pay patients. Insurance and Medicare restrictions apply.
